Primary care networks (PCNs) have been introduced to south east London. These networks connect practitioners with community services, social care and other providers of health and care services to provide better support and care to local patients.

This means your primary care team is part of a wide network of practitioners and specialists dedicated to ensuring you receive the best care possible. These changes allow you to receive the support and care required from different professionals in your community, whilst freeing up doctors’ time to help treat the sickest patients.
